Dental Direct Billing

Each patient is valued as an individual in our office, which is why we adapt each visit to meet your personal dental needs.

We accept most dental insurance.

Private and Employee Group Plans: including but not limited to Alberta Blue Cross, Great-West Life, Green Shield, Manulife and Sun Life dental insurances.
Government and Provincial Dental Coverage: including Child Health Benefits, ADSC (Alberta Government dental programs), and Alberta Seniors Benefit.

 

We help with Direct Insurance Billing

With your consent, 5th Avenue Dental Centre submits your dental claim directly to your insurance provider after you receive your treatment. Your claim is sent electronically and a claim response will be issued. No longer do you have to deal with the hassle of submitting paperwork; paying the full amount out-of-pocket; or waiting for reimbursement.

Depending on your dental coverage, there may be a patient co-payment portion. For example, if your plan covers 80% of the cost of treatment,  you will pay the outstanding balance of 20% at the end of your treatment. For any services or treatment not covered by insurance you may pay using Cash, Check, Debit, Visa or Mastercard.

If your dental plan does not allow benefit assignment, we will still submit the claim for you and you will be reimbursed under the terms and conditions of your dental plan.
